REGISTRATION POLICY

All New Patients have to complete Patient Registration 3 days before the appointment or within 8 hours
of booking one if your appointment is coming up in less than 3 days.

INSURANCE INFORMATION

Sometimes, the information that you have provided us with maybe incorrect or we may require
additional information to verify your benefits. If we contact you about your insurance, we ask that you
get back to us as soon as possible.
*We reserve the right to cancel any New Patient appointment without proper coverage information.

CONFIRMING YOUR FIRST APPOINTMENT

We require that all patients confirm their appointments promptly. The 1st visit must be confirmed 7 days
in advance or at the time of booking if the appointment is within the next 7 days.

APPOINTMENT CONFIRMATION POLICY FOR RETURNING PATIENTS

We reserve the right to cancel any appointment that is not confirmed at least 48 hours before the
appointment time.

GRACE PERIOD

Our grace period is 10 minutes.
We understand that different circumstances can affect your arrival at the office. However, to ensure that
all the patients are seen on time we cannot extend the courtesy period.

X-RAYS POLICY

When visiting the dentist for an exam, consultation, or dental treatment, it is necessary to have dental
X-rays taken. If your insurance limits the frequency of X-rays, we may ask you to provide them from
another office or pay for new ones out of pocket. Without the x-rays on file during your visit, we will not
be able to see you.
Since our hygienists operate under the supervision of our dentists, we require that all patients have
X-rays at least once every two years and exams at least once every year.

CANCELATION POLICY

Because your appointment time is reserved only for you, 24 hours is required in the event you need to
cancel your appointment.
Cancellations made with less than 24 hours notice will result in a BROKEN APPOINTMENT FEE.
Patients who do not show up to a scheduled appointment without a call to cancel will be considered as
“no show.” No show appointments are also subject to the cancellation fee.
● As a courtesy we will waive cancellation fee for the 1st missed appointment.
● Cancellation fee for the 2nd missed appointment is $75.
● If 3 appointments have been missed consecutively, the cancellation fee for 3rd missed
appointments will be $150.
Please be advised that we will not be able to reschedule your appointment unless cancellation fees are
paid.
We reserve the right to dismiss patients if 3 appointments have been broken.
