Sleep apnea CPAP vs. Dental appliance

According to many studies, though CPAP machine is considered to provide a better solution for patients with Severe Sleep Apnea, due to many restrictions, discomforts and numerous side effects patients experience while wearing CPAP machine, many patients end up switching to Sleep Apnea mouth guards because of the comfort and ease of use.

How does the Sleep Apnea mouth guard work?

To stop the apneic events, the Sleep Apnea Oral Appliance is designed to protrude the lower jaw forward enough that it brings your tongue forward and out of the way of blocking your airways. The appliance will protect your teeth from bruxing or clenching. As a result, the airway opening of your mouth will be fuller to enable proper breathing and for the Sleep Apnea events to cease.

Another way to determine if you are having sleep issues is by going to your dentist. That may sound strange, but your dentist is the one doctor you visit that looks in your mouth all the time. Since Sleep Apnea is related to having a large tongue, large uvula, large tonsils, scaled tongue/tongue thrusting, and wear and tear on your enamel a dentist trained in diagnosing and treating Sleep Apnea would be the first person to question whether you get a good night sleep after your bed partner or roommate.

209 NYC dentists and staff are trained in diagnosing, testing and treating Sleep Apnea.

Sleep Study and Ѕlеер Арnеа Теst

Sleep Study in a lab

There are two options available to the patients, one of them is to be tested in a sleep clinic. We describe this testing process below.

Home Sleep Study Test

The second option, gaining more popularity is Home Sleep Test. Many patients prefer this option due to several reasons:
Patients are tested while they sleep in the comfort of their home, on their own bed.
The cost of the Home Sleep Test is significantly lower. Whether patients have medical insurance or not Home Sleep Test is a very affordable option, that will be reimbursed by your insurance the same way as the clinic test.
The results of the Home Sleep Test are interpreted by the same Board Certified Sleep Specialists.

209 NYC Dental is happy to be able to provide our patients with the ability to have Home Sleep Test.

Sleep Apnea Diagnosis, Testing, and Treatment at 209 NYC Dental

Sleep Apnea Screening and Free Consultation

If you are a current dental patient of ours, the sleep apnea consultation and examination can take place after you’ve had your adult checkup and teeth cleaning or after most scheduled dental procedures. If you are new to us, please schedule a free consultation for your initial consultation and screening.

Here’s what to expect at your Complimentary Screening and Consult.
Our sleep apnea coordinator-technician will ask you to complete Epworth questionnaire. This preliminary inquiry will be able to reveal a bit more about your symptoms, history, and what category of apnea you might have. Upon its completion, you and our technician will then go over together the questionnaire and your history of sleep patterns. The technician or the doctor will further examine your mouth to determine defining factors and evidence of a possible sleep disorder. They will measure your neck and study your face and bone structure.

Rhinometer and Pharyngometer testing

If the sleep technician determines the need for further observation, she will suggest a Rhinometer and Pharyngometer testing.
The Rhinometer test measures the airflow of your nasal cavity with a wand. As you breathe in and out from each nostril into the wand, it displays on the monitor if there is any nasal airway blockage, called Upper Airway Resistance Syndrome (UARS). However, if it is discovered that your nasal airways are critically obstructed, you would be advised to see an ENT doctor before performing treatment for Obstructive Sleep Apnea. If the nasal airways remain untreated, you will continue having sleep apnea symptoms even though the obstructive sleep apnea is managed.

After the Rhinometer analysis, the Pharyngometer test follows. This time you will be breathing into a wand through your mouth. The technician will direct you to do a series of breaths in various patterns to find out if the obstructive collapse point in your lower airway (throat) is the cause of your apnea. If there is a notable blockage in your lower airway that can be seen on the monitor, you will be advised to proceed with a Sleep Home Test with our Watermark Home Sleep Testing Device.

Home Sleep Test

Unlike the sleep tests performed in sleep clinics, the Home Sleep Testing Device from Watermark Medical is a more comfortable and comprehensive way of sleep testing. This device is worn on the head with attached nasal cannulas. The technician will set up the machine with your demographics information from the Epworth questionnaire and will explain how to wear the Home Sleep Test (HST) properly.

The Epworth questionnaire along with the final results of your home sleep test will be sent over to a Board Certified Sleep Specialist who interprets the data. Within 48 hours the Sleep specialist delivers a prescription with the findings, diagnoses, and treatment options (CPAP, Surgery, Appliance) back to us.

Sleep study review

A comprehensive review of the Sleep Test results is done together with your dentist and the sleep coordinator.
If the dental sleep appliance is a recommended treatment, your dentist will proceed with fabricating custom Sleep Apnea Oral Appliance.

If CPAP machine or surgery are recommended, you will be referred out to either a cardiologist or an ENT doctor. All of your findings, study results, and a prescription will be forwarded to the doctor.

Preparation of Sleep Oral Appliance

Impressions and records for Sleep Apnea Oral Appliance can and should be taken the same day your receive your results. These include study models (either actual molds or digital 3D scans), and measurements for both horizontal and lateral movement of your mouth. The impressions and records will be then sent to a laboratory. The delivery (insertion) of the custom Sleep Apnea mouth guard takes place in three weeks.

Delivery of the Dental sleep appliance

During insertion visit of your Sleep Apnea oral appliance, the dentist may perform some adjustments to the device, i.e., to further protrude or push back the lower part, to ensure comfortable fit of your Sleep Apnea dental appliance. The dentist or technician will show you how to use your dental sleep appliance.

You may be asked to come back for 2 or 3 additional adjustments within a 2-3 month span.

If no adjustments are necessary, we will ask you to keep a sleep diary to monitor any changes in your sleep.

Followups

Six months follow up at the dental office is recommended. A follow-up sleep study with you wearing your appliance may be suggested
to confirm your progress.

Can’t say enough good things about this place. We found this place because my partner had a dental emergency and no insurance. We both have dentist phobias that have prevented us from keeping on top of our teeth cleanings etc. and so we don’t have a regular one to go to in situations like this. I looked for the highest rated dentist in the city on Google and found this place, which mentions specialty in caring for emergencies on their site. And so I got him in the door based on that. They spoke the truth! They were so kind, patient, caring and non judgmental for my partner, it made me want to go in for my own cleaning and care. Now we’ve referred several people since! I personally have been seeing Dr. Reddy, who is a gem of a human who seems to enjoy her work. my friends have seen other dentists here and we’ve all had the best experience here and think ours is the best. :) Since it was years since my cleaning, I did need some work done over the last couple months or so and I made it to all of them. Instead of anxiety and maybe occasionally postponing as I would do in the past, I almost looked forward to my appointments. They made all the costs and insurance coverage completely clear and transparent and gave me choices. I commute from Bushwick but it’s been so worth it. If you are looking for the dentist, this is your place!
